The Visit Check List

will give you a detailed list of what is required for your visit. Please print this out for yourself so you can be sure to have the appropriate information and documentation.

The Check-In Sheet

includes all pertinent owner and dog information. If there is a co-owner you may (if you want) fill out a second form with the co-owners information but please note this is the co-owner’s information.

The Appendix A (Contract) forms list the specific dog(s) that are to be collected and have their semen frozen. Most often only one or two dogs will be collected so use the form titled Appendix A (Contract) Dog 1 – 3

The Telephone Authorization (Contract)

gives us specific information as to the individuals that you will grant access to or any decision making as far as the frozen semen is concerned. It does involve the use of a password of your choosing. We will not disclose any information about your semen unless the individual that is calling give us the password. Please be sure to keep these documents in a safe place. If you forget the password or would like to make any changes in the representatives; please send us in writing a signed letter stating this and a new Telephone Authorization (Contract)

filled in with the new password. This document can be electronically signed. Click on the “signature” tab then follow the on-screen instructions to apply your digital signature. If for any reason this function will not work for you send the form unsigned and we will have you sign it at the time of your visit.
